ABSTRACT

The appropriate needle device is crucial for obtaining the curative effect of fire needling therapy. The article introduces the material specification, clinical operation, indications, characteristics and advantages of the contemporary traditional fire needling devices (e.g. He's fire needle and Shi 's fire needle) and the contemporary new-type ones (e.g. fire needling with filiform needle and micro-needle); and determines the innovations of modern fire needling. It is anticipated that the needle specifications, production process and operation standard of fire needling devices should be further unified so as to provide the references for the selection of fire needling devices in treatment based on clinical syndrome differentiation and expand the clinical application of fire needling therapy.

ABSTRACT

OBJECTIVE@#To observe the effect of fire needling on prostate symptoms, quality of life, average daily number of nightly urination, urine flow rate and prostat volume in patients with mild to moderate benign prostatic hyperplasia (BPH) of kidney yang deficiency.@*METHODS@#A total of 60 patients with mild to moderate BPH of kidney yang deficiency were randomly divided into an observation group (30 cases, 3 cases dropped off) and a control group (30 cases, 4 cases dropped off). The observation group was treated with fire needling at Guanyuan (CV 4), Shuidao (ST 28) and Qugu (CV 2) twice a week (2-3 d interval between each treatment), continuous treatment for 4 weeks. The control group received lifestyle advice and education, once a week for 4 weeks. In the two groups, the international prostate symptom score (IPSS), the quality of life (QoL) score and the average daily number of nightly urination were observed before treatment, after treatment and during the follow-up of the 4th week; the urinary maximum flow rate (Qmax), the average flow rate (Qave), and the prostate volume were assessed before and after treatment in the two groups. The safety was observed in the observation group.@*RESULTS@#After treatment and during follow-up, the IPSS scores, QoL scores, and the average daily number of nightly urination in the observation group were decreased compared with those before treatment (P<0.05), and those in the observation group were lower than the control group (P<0.05). After treatment, there was no significant difference in Qmax, Qave and prostate volume between the two groups and within the each group (P>0.05). There were no fire needling-related adverse reactions, and no obvious abnormality was found in urine routine and coagulation function tests before and after treatment in the observation group.@*CONCLUSION@#Fire needling can improve lower urinary tract symptoms and quality of life, reduce frequency of nightly urination in patients with mild to moderate BPH of kidney yang deficiency, and has good safety.

ABSTRACT

The evaluation of balance includes clinical observation, scales and instrumental measures. Functional Reach Test is simple and can be carried out in both standing and sitting, but the error of reading the measuring ruler is large, which results in new moving rulers and inertial sensors. The factors influencing the results of Functional Reach Test are moving strategy, age, moving efficiency, goal orientation, single or double arms, human characteristics, number of experiments and others. In the future, combination of electromyogram and inertia sensor can be used to discuss the variety of muscles and the changes of muscle strength, and more influence factors for the test are needed to research.

ABSTRACT

Objective:To analyze the effects of proprioceptive neuromuscular facilitation (PNF) on balance, motor and activities of daily living (ADL) for stroke patients. Methods:The randomized controlled trials about the effects of PNF on motor, balance and ADL in stroke patients were retrieved from PubMed, Embase, CNKI, Wanfang data, and VIP, since establishment to December, 2019. The primary outcome measures were Berg Balance Scale (BBS), Fugl-Meyer Assessment (FMA) and modified Bathel Index (MBI). The secondary outcome measures were Functional Reach Test (FRT), Trunk Impairment Scale (TIS), and Timed 'Up and Go' Test (TUGT). The literatures were independently screened by two investigators, and the quality of the articles was evaluated using the Cochrane Library systematic review criteria, and meta-analysis was performed using RevMan 5.3. Results:A total of 13 articles were included with 786 stroke patients. PNF increased the score of BBS for sequelae patients (MD = 3.31, 95%CI 2.58 to 4.04, P < 0.001), FMA for recovery patients (MD = 8.31, 95%CI 5.68 to 10.97, P < 0.001); as well as MBI score (MD = 6.84, 95%CI 5.20 to 8.48, P < 0.001), FRT distance (MD = 1.11, 95%CI 0.39 to 1.84, P = 0.003) and TIS score (MD = 1.75, 95%CI 1.19 to 2.31, P < 0.001) for all the patients, and decreased TUGT time for sequelae patients (MD = -1.86, 95%CI -2.62 to -1.10, P < 0.001). Conclusion:PNF can significantly improve balance, motor and ADL for stroke patients. The effectiveness is various with the course of disease. More high-quality researches are needed.

ABSTRACT

Objective:To explore the effects of home-based telerehabilitation guidance on physical function and activities of daily living for stroke patients. Methods:From September, 2015 to March, 2018, 240 stroke recovering patients from three community health service centers in Fengtai District, Beijing for outpatient rehabilitation were randomly divided into control group (n = 120) and tele-rehab group (n = 120). Both groups accepted routine rehabilitation for six months, while the tele-rehab group received telerehabilitation guidance in addition. They were assessed with Fugl-Meyer Assessment (FMA) and modified Barthel Index (MBI) before and after treatment. Results:The scores of FMA and MBI improved after treatment in both groups (|t| > 14.536, P < 0.001), and improved more in the tele-rehab group than in the control group (|t| > 2.200, P < 0.05). Conclusion:Home-based telerehabilitation guidance may enhance the effect of community-based outpatient rehabilitation for stroke patients.

ABSTRACT

Objective:To investigate the current status of coping behaviors among caregivers of children with disabilities in the context of rehabilitation and its influencing factors. Methods:From December, 2019 to January, 2020, and August to September, 2020, a total of 358 caregivers (parents) of children with disabilities were surveyed in Shanghai. They were evaluated in the context of rehabilitation of children with disabilities with Coping Health Inventory for Parents (CHIP). Multiple linear stepwise regression analysis was used to explore the influencing factors. Results:The total score of CHIP was (106.00±23.45) and the average score of CHIP was (2.36±0.52). Among the caregiver factor, mothers (B = 7.607, 95%CI 2.477 to 12.737, P = 0.004), and caregivers who didn't need to care for others (B = 5.758, 95%CI 1.174 to 10.343, P = 0.014) reported higher CHIP score. Among the child factors, higher CHIP score was reported in caregivers who had more partners (B = 2.925, 95%CI 1.233 to 4.618, P < 0.001), participated in public space activities more frequently (B = 3.906, 95%CI 1.743 to 6.068, P < 0.001) and perceived changes in rehabilitation outcomes (B = -16.832, 95%CI -31.471 to -2.193, P = 0.024); CHIP score was lower in thoses of 3 to < 6 years old (B = -7.914, 95%CI -15.157 to -0.671, P = 0.032) and with intellectual disability children (B = -11.153, 95%CI -21.360 to -0.947, P = 0.032). Among the environmental factors, caregivers who perceived more friendly social attitude (B = 2.560, 95%CI 0.615 to 4.505, P = 0.010) reported higher CHIP score. Conclusion:It is proposed to learn the coping needs of caregivers of children with disabilities in the context of rehabilitation. The coping behaviors of caregivers are influenced by multiple factors, such as children's individual, caregivers and environment.

@#AIM:To observe the clinical effects of YAG laser ablation in patients with physiological vitreous floaters by A/B ultrasound combined with autofluorescence(AF)of angiography.<p>METHODS: Screening of 30 patients(30 eyes)with physiological vitreous floaters from August 2018 to January 2019 in Hankou Aier Eye Hospital, we used A/B ultrasound to measure the distance from the wall of the ball and the back surface of the lens before operations. Check the patient's best corrected visual acuity, non-contact intraocular pressure, scanning laser fundus photography, A/B ultrasound and autofluorescence(AF)of angiography before treatment, laser ablation was performed by the same skilled doctor using ELLEX YAG laser ablation machine. After treatment, the patients were followed up at the point of 1d, 1wk, and 1mo. The same examination was performed after operation. The Image-J software was used to measure the vitreous opacity in the autofluorescence of angiography pictures before and after treatment.<p>RESULTS:There was no significant difference in the best corrected visual acuity and non-contact intraocular pressure before and after treatment(<i>P</i>>0.05). Of the 30 eyes, 28 eyes underwent laser ablation treatment once, and 2 eyes underwent laser ablation treatment twice. The average number of treatments was 1.06. After treatment, the autofluorescence of angiography showed that the vitreous opacity was obviously reduced or disappeared, and the difference was statistically significant before and after treatment(<i>P</i><0.01). No complications occurred during or after surgery.<p>CONCLUSION: A/B ultrasound combined with autofluorescence confirmed that Nd:YAG laser is safe and effective in the treatment of physiological vitreous floaters.

ABSTRACT

Objective To learn the status and influencing factors of health literacy among Shanghai farmers, providing references for further health literacy intervention. Methods Subjects were selected whose occupation was filled in as farmer in health literacy monitoring of Shanghai residents in 2018. Results Among the 4 380 farmers selected, 1 878(42.88%) were males and 2 502(57.12%) were females.The overall level of health literacy among farmers was 13.42%, the proportion of their basic health concepts and knowledge, healthy lifestyle and behavior, and basic health skills being 22.28%, 15.53% and 14.45% respectively.In addition, they had health literacy in six aspects:attitude towards health, safety and first aid, health information, chronic diseases prevention and control, basic medical care and infectious diseases prevention and control; their proportions were 40.53%, 39.84%, 21.67%, 18.33%, 15.80% and 15.73%, respectively.Logistic regression analysis showed that the region and education level were the influencing factors of the health literacy among farmers. Conclusion The health literacy of farmers in Shanghai is lower than that of the city residents, and their literacy of basic health skill, primary medical care and infectious disease prevention needs to be improved.

ABSTRACT

Bobath concept improves walking and motor function in stroke patients with hemiplegia by promoting posture control and establishing correct body mapping. The use of the Bobath concept alone or in combination with other treatments, such as knee stability training, Botulinum toxin injection, and Traditional Chinese Medicine, can effectively improve the walking ability of stroke patients. The Bobath concept has not been widely used, and its clinical effect is difficult to assess, which is the major problem at the current stage in China.

ABSTRACT

Objective] To evaluate effect of health literacy intervention in occupational groups , explore suitable ways and provide suggestions for workplace health promotion . [ Methods ] In application of convenience sampling , Minhang District , Hongkou District and Qingpu District each selected 2 enterprises respectively according to their type , scale and location , setting one as intervention enterprise and the other as control .Then a six-month comprehensive intervention was conducted among intervention groups .Two hundred employees in each enterprise were randomly investigated before and after intervention to evaluate the effect . [ Results ] The overall health literacy level of participants in the intervention group rose from 16.7% to 33.4%, with improvement on the literacy levels of scientific view , infectious diseases prevention and treatment , chronic disease control and prevention and the three aspects of health literacy content ( P<0 .05 ) , while such improvement was not reflected in the control group , and health skill literacy, safety and first aid literacy declined (P <0.05). [Conclusion] Comprehensive intervention based on health needs could improve the level of health literacy of occupational groups . Intervention in future should focus on improving the literacy of chronic disease control and prevention and basic medical care in occupational population .

ABSTRACT

<p><b>OBJECTIVE</b>To explore the relationship between the strength of tobacco control and the effectiveness of creating smoke-free hospital, and summarize the main factors that affect the program of creating smoke-free hospitals.</p><p><b>METHODS</b>A total of 210 hospitals from 7 provinces/municipalities directly under the central government were enrolled in this study using stratified random sampling method. Principle component analysis and regression analysis were conducted to analyze the strength of tobacco control and the effectiveness of creating smoke-free hospitals.</p><p><b>RESULTS</b>Two principal components were extracted in the strength of tobacco control index, which respectively reflected the tobacco control policies and efforts, and the willingness and leadership of hospital managers regarding tobacco control. The regression analysis indicated that only the first principal component was significantly correlated with the progression in creating smoke-free hospital (P<0.001), i.e. hospitals with higher scores on the first principal component had better achievements in smoke-free environment creation.</p><p><b>CONCLUSIONS</b>Tobacco control policies and efforts are critical in creating smoke-free hospitals. The principal component analysis provides a comprehensive and objective tool for evaluating the creation of smoke-free hospitals.</p>

ABSTRACT

Objective To investigate the changing trend of mortality and the spectrum regarding causes of death in the population of Hunan, and to analyze the health-related major diseases.Methods With retrospective study method, a sample survey on causes of death and the related information was carried out from 2004 to 2005 among the residents in Hunan province. Results were compared with the data from a retrospective survey on causes of death in Hunan during 1973-1975 and 1990-1992, respectively. Results The crude mortality rates and the standardized mortality ratios (SMR) of the reisidents in Hunan were 901.59/100 000 and 865.14/100 000 during the period of 1973-1975 while 588.64/100 000 and 537.42/100 000 druing the period of 1990-1992, and 608.27/100 000 and 413.67/100 000 during the period of 2004-2005, respectively. During the past 30 years, the SMR of the residents in Hunan decreased by 52.18% and the descending range from the rural areas was more than that of the urban areas, and higher in females than in males. The death proportion of infectious diseases, maternal and perinatal diseases, nutritional deficiencies decreased significantly (P<0.01), but the death proportion of chronic non-communicable diseases increased significantly (P<0.01). The changing ranges of the former two proportions were both larger in the rural areas than those in the urban areas. The highest proportions of deaths due to injury and poisoning during the past 30 years were both seen in the 5 year olds, followed by the age groups of 25 year olds (P<0.01). During the period of 2004-2005, the death proportion of all the infectious diseases,maternal/perinatal diseases and nutritional deficiencies was 8.01% altogether, and those of chronic non-communicable diseases, injury and poisoning were 80.66% and 11.33%, respectively. During the past 30 years, the SMR of the three kinds of diseases all significantly decreased, more significantly seen in infectious diseases, maternal and perinatal diseases, and nutritional deficiencies but less in chronic non-communicable diseases. Among the death cases of chronic non-communicable diseases,the SMR of cerebrovascular disease, malignant tumor, and heart disease showed an increasing tendency and the sequences of them had been advanced to the first, the second, and the fourth in the death ranking during the period of 2004-2005, respectively. Conclusion The SMRs of the residents in the urban and rural areas from Hunan province showed a declining tendency.Cardiovascular, cerebrovascular diseases and malignant tumors had become the important diseases affecting the health of the people, while injury and poisoning had otherwise topped the causes of death among children and adolescents in Hunan province.

ABSTRACT

<p><b>OBJECTIVE</b>To probe the therapeutic effect and safety of ginger-salt-partitioned moxibustion on urination disorders poststroke.</p><p><b>METHODS</b>Eighty-two cases of cerebral apoplexy, including cerebral infarction and cerebral hemorrhage, with urination disorders induced by neurogenic bladder at restoration stage were randomly divided into a treatment group treated with ginger-salt-partitioned moxibustion at Shenque (CV 8) and routine acupuncture, and a control group treated with routine acupuncture. Thirty-nine cases in the treatment group and 36 cases in the control group completed all treatments. The treatment was given 5 times each week and the therapeutic effects were observed after treatment of 3 consecutive weeks.</p><p><b>RESULTS</b>Ginger-salt-partitioned moxibustion in improvement of mean urination times of each day, mean times to be asked to awaken for the nursing personnel at night, mean times of urgent urinary incontinence at day for the patient, cases-times of urinary incontinence of the patient at night, and increasing degree of urinary incontinence was better than the control group (P<0.01, P<0.05).</p><p><b>CONCLUSION</b>Ginger-salt-partitioned moxibustion is a safe and effective therapy for urination disorders poststroke.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To observe the therapeutic effect of He's Santong needling method on neurological functional deficit in the patient of ischemic apoplexy, and search for an effective method for treatment of ischemic apoplexy.</p><p><b>METHODS</b>According to the randomized and multi-central trial principle, 319 cases enrolled were randomly assigned to an observation group of 161 cases and a control group of 158 cases. The observation group were treated with He's Santong needling method, i. e. blood letting, puncturing with a red-hot needle and filiform needle therapy by stages, and the control group were treated with acupuncture at 12 points of the hand and foot routinely selected in the department, once daily. After treatment of 30 days, changes of neurological functional deficit score were investigated in the two groups.</p><p><b>RESULTS</b>The total effective rate was 91.93% in the observation group and 70. 25% in the control group with a significant difference between the two groups (P<0.05); there was no significant difference in changes of the neurological functional deficit score between the patients at the acute stage and the convalescence (P > 0.05).</p><p><b>CONCLUSION</b>The therapeutic effect of He's Santong needling method on ischemic apoplexy at acute stage or convalescence is better than that of the routine point selection.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To observe the effect of "Three ways" method of acupuncture on serum levels of tumor necrosis factor-alpha (TNF-alpha) and interleukin-1beta (IL-1beta) in patients with acute ischemic cerebrovascular diseases (ICVD).</p><p><b>METHODS</b>Sixty patients with acute cerebral infarction were randomly divided into the treated group treated by "Three ways" method of acupuncture and the control group by hand and foot acupuncture on acupoints of the hands and feet. Serum levels of TNF-alpha and IL-1beta were detected with radioimmunoassay before treatment and at the 6th, 12th and 28th days of treatment. Neurological deficit score (NDS) and Barthel Index (BI) were also observed in the same time.</p><p><b>RESULTS</b>After treatment, serum levels of TNF-alpha and IL-1beta in both groups decreased significantly (P < 0.05), approaching the normal range at the 28th day of treatment, which were even lower in the treated group than those in the control group, and the decreasing trend were significantly different (P < 0.05). Compared with the control group, the effect in decreasing NDS and increasing BI in the treated group was better than that in the control group (P < 0.05).</p><p><b>CONCLUSION</b>"Three ways" method of acupuncture could reduce serum levels of TNF-alpha and IL-1beta, alleviate injury of nervous function so as to improve the daily life ability, superior to hand and foot acupuncture.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To observe therapeutic effect of acupuncture on no-premonitory migraine at remission stage.</p><p><b>METHODS</b>The scores of pain extent, attack times, lasting time and accompanied symptoms of headache were investigated before treatment and after treatment of 4 weeks.</p><p><b>RESULTS</b>After treatment of 4 weeks, the scores of all items had significant changes (P < 0.01). The total effective rate was 95.0%.</p><p><b>CONCLUSION</b>Acupuncture has obvious analgesic effect on no-premonitory migraine at remission stage.</p>
